How may I request a personalized Optica Member Certificate?
Visit the Login page ("Login" access is located at the top blue bar of every page), click on the “Membership” tab and complete the certificate request form. Please allow at least 3-4 weeks for receipt of your personalized member certificate signed by the Optica CEO in the mail (timing varies by location).
